Pseudomyxoma peritonei secondary to appendiceal mucocele: a case report with characteristic imaging findings
Pseudomyxoma Peritonei (PMP) is a rare disease characterized by progressive accumulation of mucinous material within the peritoneal cavity, most commonly originating from appendiceal mucinous neoplasms. We report a case of PMP secondary to appendiceal mucocele, emphasizing its imaging characteristics, surgical management, and postoperative outcome. A 62-year-old man presented with a six-month history of progressive abdominal pain and distension. Laboratory tests revealed elevated inflammatory markers and increased CA 19-9 level. Ultrasonography demonstrated diffuse microcystic lesions and cystic dilatation of the appendix consistent with appendiceal mucocele. Contrast-enhanced computed tomography showed widespread low-attenuation mucinous ascites with characteristic hepatic scalloping and peritoneal involvement. The patient underwent cytoreductive surgery including right hemicolectomy, peritonectomy, splenectomy, and cholecystectomy. Histopathological examination confirmed high-grade appendiceal mucinous neoplasm associated with serosal mucinous adenocarcinoma. Postoperative follow-up imaging at one year demonstrated regression of intra-abdominal mucinous collections. This case highlights the importance of imaging in the early diagnosis, staging, and surgical planning of PMP. Recognition of characteristic radiological findings is essential for timely management and improved clinical outcomes.
